Insurance & Payment Options

MindMorph accepts most major insurance plans for psychiatric evaluation, medication management, and therapy, in network with carriers in Colorado and Oregon (the full lists by state are below). Spravato (esketamine) is often covered because it is FDA-approved. Ketamine-assisted psychotherapy is off-label and self-pay, with superbills for possible out-of-network reimbursement. The first consultation is complimentary, and HSA and FSA cards are accepted.

(Coverage and clinical experiences vary by individual. MindMorph assists with insurance processes but cannot guarantee specific outcomes or insurer decisions.)

Insurance accepted, by state

In Colorado, MindMorph is in network with Aetna, Anthem Blue Cross Blue Shield (through Carelon), Cigna, UnitedHealthcare including Optum behavioral health, Health First Colorado (Medicaid), Colorado Access, TRICARE West and the VA Community Care Network (both through TriWest), and others. In Oregon, the list includes Aetna, Cigna, UnitedHealthcare, Regence BlueCross BlueShield of Oregon, Providence Health Assurance, TRICARE West and the VA Community Care Network (both through TriWest), and Moda Health for plans effective December 1, 2026. The complete, current list for each state is kept below, and benefits are verified before your first appointment. The list grows as credentialing completes.

Veterans and military families welcome

I am in network with TRICARE West through TriWest, and I am a preferred provider in the VA Community Care Network, which TriWest also administers in Colorado and Oregon. If you are a veteran, the VA approves community care before the first visit, so start with your VA care team and ask for a referral to me by name. I am glad to walk you or a family member through those steps before you call anyone.

MindMorph is an independent practice. Network participation is not an endorsement by the Department of Veterans Affairs, the Department of Defense, or TRICARE.

Spravato (esketamine) coverage

Spravato is FDA-approved for adults with treatment-resistant depression and for depressive symptoms in major depressive disorder with acute suicidal thoughts or behavior. Because of that approval, many insurance plans help cover the cost. MindMorph's administrative team supports you by assisting with prior authorizations and benefit reviews, coordinating with payors to clarify coverage, and providing appropriate coding and documentation to support timely reimbursement. While MindMorph facilitates this process, final coverage decisions rest with each insurer. Details of the treatment itself are on the Spravato page.

Psychiatry and medication management coverage

Most insurance plans include benefits for psychiatric evaluation and medication management. MindMorph is in network with several carriers and provides superbills for possible out-of-network reimbursement. Staff review deductibles, copays, and mental-health benefits to the best of their ability so you can make informed choices. If your plan does not fully cover psychiatric services, flexible payment plans are available to keep treatment going. See medication management.

Ketamine and integrative services

Sublingual ketamine and ketamine-assisted psychotherapy (KAP) are off-label treatments and are typically not covered by insurance. MindMorph offers transparent pricing, detailed superbills for possible reimbursement, and flexible payment options. Many Lakewood and Denver-area patients use insurance for therapy or psychiatry and self-pay for ketamine sessions, which keeps one treatment plan intact. Rates are in the table below; the treatment is described on the ketamine therapy page.

Therapy and counseling coverage

Most insurance plans include benefits for outpatient therapy. MindMorph bills therapy under standard mental-health CPT codes and accepts PPO, HSA, and FSA payments. Benefits are reviewed before your first session, with a note on which services may qualify for partial or full coverage. See therapy and counseling.

Telehealth coverage

Many insurance plans include telehealth coverage for psychiatry and therapy. MindMorph provides both in-person and virtual care across Colorado and Oregon, and the same benefits review applies to a video visit as to an office visit.

Using your out-of-network benefits

If MindMorph is not in your insurance network, you may still qualify for partial reimbursement. A superbill is an itemized statement with CPT codes, diagnostic information, session details, and payment details; you submit it to your insurer's claims department yourself. Reimbursement is determined by your plan and insurer; MindMorph cannot influence or predict coverage outcomes.

Payment and financing options

For services not covered by insurance, MindMorph accepts major credit and debit cards, HSA and FSA accounts, and third-party healthcare financing when available. Monthly payment options exist for self-pay services. Before starting treatment, you receive a written cost estimate and a clear explanation of charges.

Combining insurance and self-pay

Yes, you can. Some patients use insurance for covered treatments, such as therapy or Spravato, and self-pay for others, such as ketamine or integrative services. The team helps you coordinate both under one plan.

Sliding scale and financial assistance

MindMorph occasionally offers reduced-rate sessions or community partnerships for patients experiencing financial hardship, on a limited basis depending on need and availability. Priority is given to individuals actively engaged in therapy or psychiatric care and meeting eligibility criteria. Availability depends on current clinic capacity; ask during your consultation.

How to verify your benefits

Text, email, or submit the online form with your insurance details. MindMorph reviews your benefits to the best of its ability and sends a summary before your appointment. The initial consultation is complimentary, a conversation to judge fit before anything else.

Areas served

MindMorph serves Lakewood, Denver, Morrison, Golden, Boulder, Highlands, LoDo, RiNo, Cherry Creek, and Aurora, and provides psychiatry and therapy virtually to residents anywhere in Colorado or Oregon through secure telehealth. The Lakewood office offers free parking and easy access from major Denver Metro routes.

A note on guarantees

MindMorph provides individualized, evidence-based psychiatric and wellness care. Coverage and reimbursement vary by insurer; MindMorph cannot guarantee third-party approvals or financial outcomes. Results cannot be guaranteed.

If you are in crisis, call or text 988 (Suicide & Crisis Lifeline) or call 911. This practice does not provide emergency services.

Questions

MindMorph is in network with several major carriers in Colorado and Oregon for psychiatry, Spravato, and therapy, including Aetna, Cigna, UnitedHealthcare, Health First Colorado (Medicaid) in Colorado, and TRICARE West in both states, with VA Community Care for veterans. Coverage varies by plan; benefits are reviewed before your appointment. If MindMorph is out of network, you receive a superbill for possible reimbursement.

Usually not. Sublingual ketamine and ketamine-assisted psychotherapy are off-label uses and are not recognized as standard covered benefits. MindMorph publishes its self-pay rates ($595 per 120-minute session, with 6- and 8-session packages) and provides superbills so you can seek out-of-network reimbursement. Many patients use insurance for therapy and psychiatry and self-pay for ketamine.

Frequently, yes. Because Spravato (esketamine) is FDA-approved for treatment-resistant depression and for depressive symptoms with acute suicidal thoughts or behavior, many insurance plans cover it. MindMorph assists with prior authorization, benefit verification, and the coding and documentation insurers ask for, but the final coverage decision rests with your insurer.

A superbill is an itemized statement that lists CPT codes, diagnoses, session details, and what you paid. MindMorph provides one after each visit. You submit it to your insurer's claims department for possible out-of-network reimbursement; many PPO plans return a portion of the cost. Reimbursement is set by your plan, and MindMorph cannot predict or influence it.

Most HSA and FSA plans allow payment or reimbursement for eligible mental-health services, including psychiatric evaluation, therapy, and Spravato. MindMorph accepts HSA and FSA cards directly at the time of service and provides itemized receipts for submission. Check your plan's rules for which services qualify, since administrators vary on the details.

Both, within limits. Monthly payment options are available for self-pay services, and third-party healthcare financing when available. Reduced-rate sessions are offered occasionally for financial hardship, on a limited basis, with priority to patients actively engaged in ongoing therapy or psychiatric care. Availability depends on clinic capacity; ask during your complimentary consultation.

I am in network with TRICARE West, the region TriWest has run since January 1, 2025, which covers Colorado and Oregon. TRICARE Prime and TRICARE Select members can see me for office-based therapy and medication management with no referral. Active duty service members are the exception and need a referral from their primary care manager first. Spravato needs pre-authorization from TriWest, and I help you request it. Your copay depends on your plan.

I see veterans as a preferred provider in the VA Community Care Network, which TriWest administers for the VA. The VA decides eligibility and approves care before your first visit. For mental health, you may qualify when the VA cannot see you within 20 days, or when the average drive to a VA clinic is longer than 30 minutes, among other criteria. Ask your VA care team for a community care referral and name me. Once the authorization arrives, you book with me, and the VA covers the services that letter lists.

That depends on how you pay. With TRICARE Prime or Select, office-based therapy and medication management with me need no referral, unless you are on active duty, when your primary care manager refers you. With VA Community Care, the VA must approve your care before the first appointment, outside of emergencies. With other insurance, I check your plan's rules when I verify your benefits, before you book.
